Refer a Patient
We love to partner with providers in our communities so that together, we can provide the best support possible for families. In order to make this process as smooth as possible, our dedicated intake specialists are always here to help.
Our Approach to Collaboration
At Bloomwell Autism Therapy, we believe that exceptional care begins with collaboration. Our treatment model embraces a team-based approach, and we partner closely with contracted related service providers—including Speech-Language Pathologists (SLPs) and Occupational Therapists (OTs),—to ensure comprehensive, coordinated care for every child. In addition, we welcome these providers into our clinics, where they have full access to our therapeutic tools, materials, and the support of a dedicated 1:1 Registered Behavior Technician (RBT) during sessions.
Furthermore, we foster ongoing collaboration between related service providers and our Board Certified Behavior Analysts (BCBAs), allowing for seamless integration of best practices across disciplines. This unified approach ensures that each patient receives the most individualized, meaningful, and effective support possible.
Individualized Assessments
Once a referral is received, we put together an individualized assessment package to help us understand how we can support each child. The results of each assessment are used to identify the deficits relative to an ASD diagnosis and inform socially significant goals for the patient and family. In addition, assessment information helps inform the clinicians if they should recommend or provide other services based on scope of competence within ABA. We use the following types of assessments to gather this information:
Norm Referenced Assessments:
• Child Family Quality of Life – Second Edition
• Vineland – 3 Comprehensive Caregiver Interview
• BASC-3 PRQ
• BASC-3 PRS
• Behavior Problems Inventory – Short Form (For school-age patients)
Direct, Criterion-Referenced Assessments:
• VB-MAPP
• VB-MAPP Milestones
• ABLLS-R
• AFLS
• Essentials for Living
• Early Start Denver Model
• PEAK
Based on the results of each assessment, our team may recommend anything from 25-40 hours of weekly therapy, depending on what the child’s needs are. If less than 25 hours are needed and our full-time program is not a good fit, then we provide a treatment plan and help each family find care that is right for them.
Support for the Whole Family
Caregivers have access to our patient portal through Central Reach, which allows them to see real-time data and session notes from their child’s therapy.
We have also developed our ROOT caregiver guidance program to provide additional tools to families. When families start the program, they learn foundational concepts that can help continue their child's growth after they leave the clinic. Once families complete those ABA 101 lessons, they are able to choose from additional topics to support their children, such as improving sleep, potty training, and building social skills.
